Participation page with Fee Agreement
and Non-Refundable Retainer and Cost Deposit
If you have a homestead in Florida or a second
home in Florida you may elect to participate in these actions.
To participate please click the "ATR" tab to access and download the
Authority to Represent (Contingency Fee Agreement) form, and click the "Questionaire"
tab to access and download our questionaire.
After these documents have been filled out and the fee agreement signed
by you, please forward us all executed documents together with your
check for either $500.00 or $250.00 now and $250 in six months
representing your non-refundable retainer and cost deposit. Please make
your check payable to Lyons and Farrar, P.A. Trust Account and
forward the signed documents and check to:

A copy of the executed fee agreement will be emailed to you. If you have
any questions about the procedure for inclusion in the litigation,
